It all starts with a plan

Whether this is organised before surgery or when preparing for discharge, our customised assessment of the supports you or your loved one needs to successfully recover builds a plan of support around the health situation and client goals. We provide only the most appropriate assistance with a combination of our personal care and companion care services to support ongoing success.

Strengthening Circles of Care

We are there when family are not able to be because of work or distance and we work with everyone involved in the care of the client – doctors, nurses, therapists – to make them more efficient.

Accessible Transportation

Regular visits to the doctors, post-operative follow-up or treatment appointments are important for successful recovery. If the client needs transportation and assistance, we can provide one or both. If they need reminding that they have an appointment or transportation to visit friends, we do that too.

Medication Reminders and Prescription Pick-ups

Appropriate use of prescribed medications leads to a better recovery. We can have prescriptions filled and collected and provide reminders so that medications are taken on time or when they are needed.

Nutrition Planning, Food Shopping and Meal Preparation

Good nutrition is essential for healing and recovery. This is more important when there are specific dietary needs, such as diabetes and heart failure. We can assist with post discharge nutrition orders and planning by preparing meals to meet specific needs. This may be best set up before discharge in consultation with the hospital’s Discharge Planner.
