Is the individual confident with online transacting with financial institutions or shops and managing their own finances?
Can the individual use a computer/ smartphone to access and participate, e.g tele health services? (or do they have a support person who will do so on their behalf?)
Does the individual have any diagnosed heart disease condition that needs ongoing monitoring /daily treatment or attention /nursing care?
Does the individual have any diagnosed diabetes that needs ongoing monitoring /daily treatment or attention /nursing care?
Does the individual have any diagnosed lung /respiratory condition that needs ongoing monitoring /daily treatment or attention?

Is the individual using any walking aids or other equipment?
Did the individual have or report any falls lately that have required medical attention?
Does the individual experience any severe pain that requires specialised assistance?
Does the individual have any vision loss or impairment recently?
Does the individual have any hearing loss recently?
Does the individual have any weight loss recently?
Does the individual need any help with dressing, having a bath/shower, eating, moving around or help with going to the toilet?
Is the individual diagnosed with any cognitive impairment such as dementia, acquired brain injury, Alzheimer's or intellectual disability that impairs reasoning, making informed decision and impact on your daily life?
Has the individual or family carer expressed any concerns that the consumer / self-experience any cognitive impairment such as memory loss, that impairs reasoning, making informed decision and impact on their daily life and their safety?
Does the individual's carer experience any form of severe stress or have a known disability or underlying condition that inhibits their ability to care for others?
Do you foresee any behavioural issues that impact on relations and the individual's daily life?
Does the individual express that they often need someone to help arrange or manage their care?
Did you observe or the indivdual express any experience physical, emotional, financial threats or felt your consumer were facing significant risk of harm?
Did the individual inform or you are aware of any active intervention order for anyone within their home? e.g. Mental health community treatment order?
Does the individual have stable accommodation?
Has the individual appointed someone as their enduring guardian and power of attorney?
Does the individual only have a single paid carer providing care to them all the time?
Are there any known restrictive practices used on the individual in home care?
Overview of restrictive practices (agedcarequality.gov.au) Chemical restraint: Using medication that restricts your movement or affects your ability to make decisions Environmental restraint: Restricting or limiting your movement, like securing doors to prevent access to outside areas or moving mobility aids out of reach. Mechanical restraint: Using a device like bed rails, low beds or clothing to prevent you from moving Physical restraint: Using force to prevent, restrict or subdue your movement Seclusion: Putting you alone in a room or physical space where you can’t choose to leave without help

Peter co-founded SH Franchising with Tony Bonacuse in 2004 and is the current CEO for Senior Helpers. He has led the organisation to become a national leader in non-medical home care in the USA. He is also the past President of the Home Care Association of America’s board of directors.
